You can create an order on line or, if you prefer, print out a copy of the appropriate order form and fax or mail it to us. You can also call us with the information. 

Phone Orders

Please call 1.800.236.0850, ext. 131, 8:00 – 5:00, Central Time, Monday through Friday if you want to talk to someone about your order. If you want to just leave your order, you can call any other time and leave a message. 

Fax Orders

Just print out the order form, complete it with your choices, and fax it to 1-608.781.3883.
